Continuum of Care

What does it take for a child to escape homelessness for good? Our Continuum of Care model means support for kids at
every step of their journey off the streets.

It begins with the homeless kids we serve

Every child’s experience and life is unique. That’s why our staff works with each kid individually
to develop a case plan that is tailored to their specific needs and goals.

What is our Continuum of Care?

Through a combination of support strategies, including educational programs, job training
and placement, medical services, mental health and substance abuse counseling, legal aid
and beyond, we help young people embrace the great promise of their lives, overcome steep
barriers to independence and strive to achieve their aspirations.

Street & Van Outreach

Our Outreach teams are often the critical first step in connecting
homeless youth with assistance. Armed with warm blankets, free
sandwiches and information about Saving Angels, they’re out on
the streets every day to help kids.

Crisis Care and intake

When kids come to our shelters, addressing their immediate needs
is our priority – warm meals, showers, safe beds. But our Crisis Care
involves so much more: It’s also about building a foundation of
safety and trust so often eroded by life on the streets.
.

Long-term support

Once a child’s most urgent needs are met at our crisis shelters, we
offer longer-term support services and transitional housing (through
our Rights of Passage program) to guide our youth on a path of self-
sufficiency.
